Work Environment\n

This position operates primarily in a remote setting, allowing you the convenience of working from home while still engaging with clients through virtual consultations. Our clinic emphasizes a collaborative approach, coordinating with various health professionals and services to ensure clients receive the holistic care they need. As part of our community health clinic setup, you will also occasionally visit clients in-person to conduct thorough assessments and facilitate ongoing care.

Key Responsibilities\n

• Conduct comprehensive assessments of client needs through telehealth sessions and in-person visits, focusing on psychosocial factors that affect seniors’ emotional and physical health.

• Develop and implement individualized care plans in collaboration with clients and their families, setting therapy goals that help facilitate their continuous progress and enhance their quality of life.

• Provide one-on-one counseling and emotional support to seniors, assisting them in navigating challenges related to aging, loss, and life transitions.

• Act as an advocate for clients, ensuring they have access to necessary community resources, programs, and services that can support their overall well-being.

• Collaborate with healthcare professionals, including nurses and caregivers, to coordinate services that improve clients’ health outcomes and address any identified concerns.

• Conduct follow-up visits to monitor evolving client needs, ensuring adjustments to care plans as necessary to maintain optimal support.

• Maintain comprehensive and accurate documentation of assessments, care plans, progress notes, and any significant interactions with clients, ensuring compliance with our established ethical standards.

This role primarily focuses on senior clients within our community, addressing their unique psychosocial needs that arise from aging. You will work with individuals who may be experiencing life transitions, mental health challenges, or require support in maintaining independence. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• Current, valid licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington State.

Compensation and Benefits\n
