Key Responsibilities\n

• Conduct in-depth psychosocial evaluations of seniors to assess their emotional, mental, and social needs in order to develop individualized care plans.

• Provide counseling and emotional support to clients and their families, assisting them in managing life transitions, such as health issues or loss of independence.

• Advocate for clients by ensuring access to community resources, services, and support networks that are essential for their well-being.

• Engage in interdisciplinary collaboration to create and enhance care strategies that align with each client’s unique circumstances and preferences.

• Monitor client progress through regular check-ins and visits, addressing any concerns regarding their living conditions or safety.

• Facilitate family discussions to resolve conflicts and improve communication related to the care of senior clients.

• Educate clients and families about available community resources, support groups, and programs that promote active engagement and quality of life.

• Maintain accurate and comprehensive documentation of assessments, care plans, and interaction notes in compliance with ethical standards and regulatory requirements.

• Stay current with best practices in social work and relevant social policies that affect geriatric populations.

Patient Population / Client Focus\n

The primary focus of this role is on the senior population, particularly those facing challenges of aging, mental health issues, and social isolation. Your expertise will assist clients in reclaiming their emotional health, encouraging them to overcome anxiety, depression, and other mental health barriers. This position also involves working closely with the family members of seniors, enhancing their understanding of the resources available and how to provide much-needed support.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Valid and current licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington state.

• At least two years of relevant experience in a social work role, with a preference for candidates who have worked in community centers or with geriatric populations.

• Solid understanding of social work principles, particularly related to aging and available community resources.

Compensation and Benefits\n
