Wound Care Director
Aliya Healthcare is seeking a compassionate and dependable Wound Care Director to join our team and lead the coordination and delivery of high-quality wound care services for our residents. The Wound Care Director plays a key role in ensuring the health, comfort, and dignity of our residents, while working alongside a skilled clinical team in a supportive environment.

The primary purpose of the Wound Care Nurse is to provide for the day-to-day care needs of the residents in a Skilled Nursing Facility environment.

• Provide hands-on care to residents based on their Plan of Care.

• Identify changes in resident condition and evaluate care needs.

• Demonstrate strong assessment and documentation skills.

• Evaluate and participate in the development of the Resident Plan of Care.

• Monitor overall condition and provide care in compliance with facility policies and procedures.

• Provide primary skin care to residents under the medical direction of the attending physician or Director of Nursing.

• Identify, manage, and treat skin disorders, lesions, and decubitus ulcers.

• Ensure appropriate treatment and prevention for residents with decubitus ulcers, including daily inspection, turning, activity, nutrition, and cleanliness.

• Remove dressings, clean wounds, apply medications per physician orders, and redress wounds.

• Perform daily dressing changes and weekly wound documentation.

• Assess, document, treat, and dress wounds accurately and consistently.

• Implement total nursing care plans through assessment, planning, and evaluation.

• Administer prescribed medications and treatments and evaluate effectiveness.

• Implement total nursing care plan through assessment, planning and evaluation.

• Administer prescribed medications and treatments according to policy and procedures; evaluate treatment effectiveness on a continuing basis.

• Assist in maintaining a physical, social, and psychological environment conducive to residents’ well-being.

• Participate in resident and family education.

• Monitor team member performance; assist in staff development, evaluation, and discipline as needed.

• Assign duties to team members and ensure completion.

• Function as charge nurse and resource for staff questions, problem-solving, and resident care decisions.

• Adhere to all facility and department safety policies and procedures.

• Stay current with facility policies, procedures, and nursing trends through in-services and education.

• Recognize significant resident condition changes and take appropriate action.

• Document all care, resident response, and necessary data per facility policies.

• Educate families, staff, and residents on nutrition, hydration, skin care, and repositioning to support wound healing.

• Ensure medical supplies are available for wound care.

• Adhere to all safety policies and procedures.

• Ability to communicate and work effectively with patients and staff.

• Ability to follow facility policies and procedures.

• Proficiency in spoken English.

• Initiate care plans and complete MDS Charting.

• Adequate vision and hearing (with or without prosthetics) to perform duties.

• Ability to work with residents who may be ill, disabled, elderly, emotionally upset, or occasionally combative.

• Ability to maintain current practice knowledge as it relates to job duties.

Qualifications

• Current and valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state.
• At least 2 years of clinical experience in a long-term care, rehab, or skilled nursing setting.
• Prior experience providing wound care in a post-acute or long-term care facility.
