Job Responsibilities
- Conduct thorough evaluations in inpatient or outpatient settings, creating tailored treatment plans for each patient
- Implement physical therapy techniques focused on improving mobility, strength, and overall function
- Work in tandem with nurses, physicians, case managers, and rehabilitation staff to ensure optimal patient outcomes
- Guide patients and their families through therapeutic objectives, transitions, and exercises to carry out at home
- Keep detailed records of evaluations, treatments, and patient progress following facility guidelines and regulations
- Engage in interdisciplinary team meetings and rounds to support cohesive care delivery
- Doctorate in Physical Therapy (DPT) or relevant equivalent from an accredited institution
- Current licensure as a Physical Therapist in Wisconsin
- CPR certification is preferred
- Additional specialized certifications (e.g., neurological, geriatric) are favorable but not mandatory
- Prior experience in physical therapy is strongly preferred
- Capable of addressing the needs of patients with complex medical histories and mobility challenges
- Excellent communication and collaborative skills are vital
- Proficiency in digital health records and clinical documentation
