Registered Nurse - LTC/SNF

Job Summary:

Responsibilities:

• 
  Perform timely and accurate assessments of residents, documenting all clinical observations.
  
  
• 
  Administer medications following proper protocols and the 5 Rights to ensure resident safety.
  
  
• 
  Execute wound care procedures and conduct assessments as needed.
  
  
• 
  Monitor residents for changes in condition, reporting significant findings to the healthcare team.
  
  
• 
  Assist CNAs with resident care and respond promptly to call lights and resident needs.
  
  
• 
  Lead on the floor by assessing situations, directing staff, and ensuring quality care.
  
  

Qualifications:

• 
  Hold a valid Wisconsin Registered Nursing license and maintain certification.
  
  
• 
  Possess a two-year nursing degree; a four-year degree is preferred.
  
  
• 
  Maintain current CPR/BLS certification.
  
  
• 
  Demonstrate a minimum of 1 year of relevant clinical experience (preferred).
  
  
• 
  Exhibit exceptional accuracy, attention to detail, and documentation skills.
