• Location: Portland, OR (4400 Northeast Halsey Street)
• Start Date: 09/26/2026
• Duration: Ongoing coverage with minimum 2 weeks per month
• Shift: Mon-Fri, 8am-5pm (considering 3-4 days/week options)
• Hours/Week: 40 hours
• Employment Type: Locum (Contract)
• Licensing: Active license, DEA required
• Experience: Geriatric care experience preferred; BC or BE in Internal or Family Medicine
• Certifications: BLS

What You’ll Do :

• Provide outpatient care for adults aged 55+ within a PACE program
• Conduct home visits to assisted living, SNFs, or private residences
• Collaborate with an 11-person interdisciplinary team
• Manage patient care primarily through electronic medical records
• Coordinate with team members for comprehensive, community-based care
• Support patients with chronic conditions typical in geriatric populations
• Ensure adherence to all clinical protocols and documentation standards

What You Need :

• Active medical license in Oregon
• DEA license
• BC or BE in Internal Medicine or Family Medicine
• Geriatric care experience preferred
• BLS certification
• Ability to drive to patient residences and work independently
