The role involves care coordination and supporting patients through discharge planning. The case manager will manage a caseload of up to 25 patients. Weekend coverage is typically required every other weekend, along with the possibility of a weekday off. This role may involve floating to nearby facilities. There is an expectation to work three out of five major holidays. Experience with health insurance models and regulations is advantageous. Candidates must have experience in working with the CMS guidelines and care management protocols. The ideal candidate will have excellent organizational and communication skills.
