Registered Nurse - Hospice

Job Description

Job Summary:

Registered Nurse provides comprehensive hospice care for patients and support for families during end-of-life journeys. The role focuses on symptom management, coordinated nursing visits, and ongoing communication with the interdisciplinary team to ensure comfort and dignity.

Responsibilities:

• Deliver intermittent skilled nursing care in the home and community-based care settings as assigned.
• Manage pain and other symptoms using appropriate hospice nursing interventions.
• Coordinate plan-of-care updates with the hospice team, including social services and counseling support.
• Provide patient and caregiver education to promote comfort, safety, and continuity of care.
• Respond to clinical needs with appropriate escalation and documentation of patient status changes.
• Support end-of-life care through compassionate care, including bereavement-aware interactions.

Qualifications:

• Hold a current, unencumbered Oregon RN license upon submission.
• Maintain active Healthcare Provider Level BLS certification.
• Prefer BSN and/or prior home health, hospice, or case management experience.
• Prefer prior broad-based nursing experience to support specialized home hospice nursing needs.
• Possess a valid driver's license and required auto insurance, and maintain a reliable vehicle in good working order.
