Mid-Level Provider Full Time Requisition ID: 1543 Department: Clinical Reports To: Director of Operations FLSA Status: Exempt Grade/Level: Master’s and/or Doctorate Degree Amount of Travel Required: Position Summary Mid-Level Providers serve as licensed independent practitioners who manage health problems and coordinate care for patients with a focus on health promotion, disease prevention, and quality outcomes. They provide direct patient services such as assessment, diagnosis, treatment implementation, and monitoring of general practice plans in collaboration with a licensed physician in a primary health care setting. Providers participate in continuous quality improvement initiatives and operate within a Patient Centered Medical Home model. Specific Job Duties and Responsibilities Provide primary medical care to all patients in accordance with state board approved certification or specialty, with a focus on preventive health measures. Develop a comprehensive plan of care for each patient, including medical history, physical examination, diagnosis, treatment, referral (including hospitalization when necessary), and ordering/interpretation of laboratory and imaging studies. Utilize available diagnostic resources such as laboratory and radiological testing in an appropriate and cost‑efficient manner. Collaborate with consulting physicians, nurses, patients, and families regarding treatment and care planning. Refer MHSI patients requiring specialty services and maintain continuity of care throughout the referral process. Attend all medical staff meetings, monthly clinic meetings, quarterly provider meetings, and participate in the community health center’s QI program. Perform additional duties as required by MHSI to achieve quality primary health care goals. Communicate concerns or necessary information to the care team and participate in daily clinic team huddles. Prescribe appropriate pharmacologic and non‑pharmacologic treatments within scope of practice and following collaborative practice agreements and MHSI policy. Provide patient, family, and community education and counseling. Consult with other healthcare team members as necessary. Formulate, initiate, and monitor patient management plans using clinical protocols and best practice. Render emergency care and treatment, including advanced cardiac life support, IV fluid administration, medication administration, and injury/wound management permissible within scope granted by MHSI Board of Directors. Maintain accurate patient records per required standards, review records regularly, and obtain physician counter‑signatures when necessary. Progress notes are expected within 3 business days and must be completed within 30 days. Implement workflows supporting the MHSI Patient Centered Medical Home model. Attend and/or complete continuing education related to this position and other topics as required by MHSI. Complete continual job training processes set forth by Mainline Health Systems, Inc. Maintain safety and infection control measures and supervise safety within the local clinical settings. Participate in safety/infection control committee training as required. Reporting Relationships The Mid-Level Provider reports to and is supervised and evaluated by the Director of Operations. Evaluation Work performance evaluations are ongoing and conducted by the Director of Operations, with formal evaluations typically performed annually but may be conducted at other times by the Director of Operations, COO, Chief Clinical Officer, or CEO. Physical Demands Frequent handling of patients and patient equipment, including lifting/carrying of 10–20 lbs (occasionally up to 51–100 lbs). Varied movement activities: standing, walking, sitting, bending, and occasional climbing or kneeling. Basic vision (near and color), hearing, and touch required for effective patient care. Physical demands may require up to 33–66% of the workday in some activities, with certain tasks performed constantly (more than 66% of the time). Qualifications Graduate of an accredited Physician Assistant or Nurse Practitioner program recognized by the State of Arkansas. Arkansas license and current certification to practice in Arkansas. Effective interpersonal skills with patients and providers. Certification in BLS and ACLS. Adequate experience to perform job duties. Prescriptive authority from the state board with active DEA registration. Proficiency with computer applications such as Excel, Word, and other electronic record‑keeping systems including electronic medical records. Ability to work in a constant state of alertness and safe manner. Additional Information All Mid-Level Providers are required to obtain and document Continuing Professional Education annually per MHSI and Arkansas licensing requirements. Appointment to the position is for the duration of the grant supporting the role, contingent upon job performance meeting Board of Directors standards.
