What You'll Do

• Screen and evaluate patients with cognitive and/or ADL physical functioning disorders.
• Develop and implement individualized care plans aligned with regulatory standards.
• Provide skilled, rehabilitative treatment including ADL retraining, therapeutic exercises, modalities, and assistive devices.
• Monitor patient progress, adjust treatments, and document outcomes accurately.
• Offer counseling and education to patients, families, and caregivers about treatment options and progress.
• Communicate effectively with the healthcare team regarding patient status and care coordination.
• Complete comprehensive discharge summaries and maintain accurate records in the electronic system.
• Support supervision of occupational therapy assistants, rehab aides, and students as needed.
• Participate in meetings, training sessions, and compliance activities to ensure quality care.

What You Need Required

• Valid Occupational Therapist license (NBCOT certification) and state license in good standing
• Experience working in a Long-Term Care or similar setting
• Strong communication and documentation skills
• Ability to work independently and as part of a multidisciplinary team

Preferred

• Prior experience with adult cognitive and physical rehabilitation
• Knowledge of regulatory and clinical practice standards for occupational therapy
