Job Details:

• Title: Physical Therapist (PT)
• Employment Type: Full-Time
• Location: Lebanon, NJ 08833
• Schedule: Hours: 8:30 AM – 4:30 PM
• Pay: $48 - $55/hr. (Based on experience) + Benefits

-> Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide therapeutic interventions to improve mobility, strength, balance, and functional independence.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and interdisciplinary care teams.
• Maintain accurate and timely clinical documentation.
• Educate patients and caregivers on treatment plans and home exercise programs.
• Ensure compliance with state regulations, Medicare guidelines, and facility policies.

-> Qualifications

• Active New Jersey Physical Therapist (PT) license.
• Graduate of an accredited Physical Therapy program (DPT or equivalent).
• Experience in Skilled Nursing Facility (SNF), Long-Term Care (LTC), Rehabilitation, Sub-Acute, or Post-Acute settings preferred.
• Strong communication and documentation skills.
• CPR/BLS certification preferred.
