Responsibilities
- Provide patient care within the case management program, supporting development, implementation, revision, and reporting.
- Act as a liaison among the patient, family, nursing staff, physicians, multidisciplinary team, and patient's healthcare benefactor to optimize outcomes.
- Serve as a consultant to the healthcare team on specific patient items and care coordination needs.
- Coordinate a continuum of care for a defined population from pre-admission through post-discharge.
- Ensure patients and families move efficiently through the course of hospitalization and transitions of care.

Requirements
- Active California RN license
- Experience in case management or care coordination (preferred)
- Strong communication and collaboration skills
- Ability to work in a hospital setting within a multidisciplinary team
- Availability to work 4x10-hour day shifts
