Roles & Responsibilities
- Provide excellent patient care by collaborating with the healthcare team in the development and implementation of the case management program.
- Act as a liaison between patients, families, and the healthcare team to optimize outcomes.
- Coordinate the continuum of care for patients from pre-admission through post-discharge.
- Ensure efficient progression of patients and families throughout the hospitalization process.
- Serve as a consultant to the healthcare team on specific patient needs.
