Case Manager – Nursing

Schedule: Days, 10 hours per day, 40 hours per week

Assignment Details:

• Contract Length: 8 Weeks
• Guaranteed Hours: 40 per week

Roles & Responsibilities:

• Provide excellent patient care by assisting in the development, implementation, and revision of the case management program.
• Act as a liaison between patients, families, healthcare teams, and healthcare benefactors to optimize patient outcomes.
• Coordinate a continuum of care for a defined patient population from pre-admission through post-discharge.
• Ensure efficient progression of patients and families through hospitalization.
