Description: The Registered Nurse implements professional nursing practice centered on the patient as a whole person. This practice focuses on creating a caring environment that emphasizes healing and wellness. Nursing practices are based on evidence and current research, as well as hospital and departmental policies and procedures. Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through supportive relationships among nursing staff and across other disciplines. The RN coordinates, ensures, and provides direct patient care activities on the unit. Responsibilities include conducting nursing rounds, assessments, and treatments. The RN facilitates admission, transfer, and discharge activities. Providing patient and family-centered education is a crucial aspect of the role. Care coordination and individualized treatment planning are essential functions. Comprehensive assessments are conducted according to the RN's scope of practice and hospital policies. The RN considers the patient’s culture, age, and developmental level during assessments. Identifying patient risk factors and presenting complaints is a key responsibility. The RN collects both objective and subjective data. The initial admission assessment is verified if obtained by other members of the healthcare team. A problem list is developed, prioritizing patient needs based on input from the inter-professional team. The RN designs actions and interventions relevant to the patient’s culture and age. Desired outcomes are determined in coordination with patients and families in accordance with the medical plan of care. Patient care therapies and procedures are fully implemented with empowerment and clinical autonomy. The RN explains procedures, treatments, and processes to patients and families. Actions to alleviate patient stress and discomfort are initiated. Changes in patient condition are recognized, responded to, and communicated appropriately. Ongoing assessments and responses to procedures and therapies are documented according to hospital policy. Tasks are delegated appropriately, and RN oversees team members in patient care activities. The RN monitors the accuracy and completeness of care provided by other team members. Educational needs of patients related to prioritized problems are identified and addressed according to understanding levels. The patient discharge process is facilitated by timely execution of discharge orders and coordination with Case Management. Input is provided into patient/nurse assignments considering staffing, unit activity, and patient acuity. The RN actively participates in unit-based activities that support organizational values and priorities. Promoting a positive group dynamic and healing environment is essential. Assuring nursing excellence according to accepted standards, evidence-based practice, and a caring philosophy is fundamental. Active participation in department and unit quality improvement activities is expected. Using a holistic perspective regarding patient care is encouraged.
