Description: The RN implements professional nursing practice centered on the patient as a whole person. The role focuses on the creation of a caring environment that emphasizes healing and wellness. Nursing practices are based on evidence and current research as well as hospital policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. The RN coordinates, ensures, and provides direct patient care activities on the unit. This includes provision of nursing rounds, assessments, and treatments. The RN facilitates admission, transfer, and discharge activities, providing patient and family-centered education. The RN is involved in care coordination and individualized treatment planning as applicable. The RN performs and documents a comprehensive assessment in accordance with scope of practice and hospital policies/procedures. Consideration is given to the patient’s culture, age, and developmental level. The RN identifies patient risk factors and presenting complaints and obtains relevant health information. Objective and subjective data are collected for assessment. The RN verifies initial admission assessments obtained by other healthcare team members. A problem list is developed, prioritizing patient needs based on input from the inter-professional team. Actions and interventions are designed relevant to the patient’s culture and age. Desired outcomes are determined in coordination with the patient and family per the medical plan of care. The RN fully implements patient care therapies and procedures with clinical autonomy within the scope of practice. Procedures, treatments, and processes are explained to the patient and family to alleviate stress and discomfort. The RN recognizes, responds to, and communicates changes in patient condition appropriately. Ongoing assessments and responses to procedures and therapies are conducted and documented according to hospital policy. Tasks are delegated appropriately, and the RN oversees team members in the performance of activities regarding patient care. The accuracy and completeness of delegated patient care are monitored. Educational needs of patients are identified and addressed related to prioritized problems according to comprehension levels. The RN facilitates the discharge process by executing discharge orders timely and ensuring proper discharge teaching. Input into patient/nurse assignments is provided, considering staffing, unit activity, and patient acuity. The RN actively participates in unit-based activities in support of organizational values and priorities. A positive group dynamic and healing environment are promoted through words and actions. Nursing excellence is assured in accordance with accepted standards and evidence-based practice. The RN actively participates in department and/or unit quality improvement activities and core measures. A holistic perspective regarding patient care is applied.
