Responsibilities
- Perform comprehensive PT evaluations for residents in a skilled nursing setting and contribute to robust care plans.
- Implement therapy interventions targeting mobility, gait, balance, strength, and functional independence.
- Monitor progress, document outcomes, and adjust treatment plans in accordance with facility policies.
- Collaborate with an interdisciplinary team to coordinate care, communicate patient status, and participate in rounds.
- Educate residents and family members on exercise programs, safety, and fall-prevention strategies.

Requirements
- Active Physical Therapist license for practice in the United States (or eligibility per applicable state requirements).
- Ability to work in a skilled nursing facility environment and adhere to facility protocols.
- Strong documentation, communication, and collaboration skills.
- Willingness to work the stated shift pattern and participate in a travel assignment.
- Proficiency with standard PT evaluation and treatment documentation processes.
