Key Responsibilities

Evaluate and treat patients in a skilled nursing facility setting to restore or maintain functional independence. Develop and implement individualized occupational therapy treatment plans based on patient needs and goals. Document patient progress, treatment sessions, and discharge planning in accordance with facility and regulatory standards. Collaborate with interdisciplinary care teams to coordinate comprehensive patient care. Educate patients and caregivers on therapeutic techniques and adaptive strategies to support daily living activities.

Required Qualifications

Graduate of an accredited Occupational Therapy program. Current and valid Occupational Therapist (OT) licensure in the state of New York. National Board for Certification in Occupational Therapy (NBCOT) certification. Experience working in a skilled nursing facility (SNF) setting preferred.

Preferred Experience / Skills

Prior experience in a SNF or post-acute care environment. Familiarity with Medicare documentation and compliance requirements in a SNF setting.
