About the Role:

The Service Coordinator for IDD (Intellectual and Developmental Disabilities) plays a critical role in ensuring that individuals with intellectual and developmental disabilities residing in nursing homes receive comprehensive, person-centered support services tailored to their unique needs. This position involves coordinating care plans, facilitating access to community resources, and collaborating with healthcare providers, families, and support staff to promote the well-being and independence of clients. The Service Coordinator acts as an advocate, ensuring compliance with regulatory requirements while fostering a supportive environment that enhances quality of life. Through ongoing assessment and communication, the coordinator monitors service delivery effectiveness and adjusts plans as necessary to meet evolving client goals. Ultimately, this role is pivotal in bridging gaps between clients and service systems to empower individuals to achieve their fullest potential within their communities. Must drive to nursing facilities using an agency vehicle.


Minimum Qualifications:

• Bachelor's degree in social work, psychology, human services, or a related field.
• Excellent communication and interpersonal skills.

Preferred Qualifications:

• Experience working with individuals with intellectual and developmental disabilities.
• Experience using Microsoft Office applications.
• Bilingual abilities or experience working with diverse cultural populations.
• Ability to manage multiple cases and maintain detailed records.

Responsibilities:

• Develop, implement, and regularly update individualized service plans in collaboration with clients, families, and multidisciplinary teams.
• Coordinate and monitor the delivery of support services, ensuring they align with client needs and regulatory standards.
• Serve as the primary liaison between clients, service providers, healthcare professionals, and community resources.
• Conduct comprehensive assessments to identify client strengths, preferences, and areas requiring support.
• Maintain accurate and timely documentation of service plans, progress notes, and compliance reports.
• Advocate for client rights and access to necessary services, including housing, employment, and healthcare.
• Facilitate meetings and communication among stakeholders to resolve issues and promote coordinated care.
• Stay informed about changes in policies, regulations, and best practices related to intellectual and developmental disabilities.

Skills:

The Service Coordinator utilizes strong organizational and communication skills daily to manage multiple client cases effectively and ensure seamless coordination among various service providers. Interpersonal skills facilitate building trust and rapport with clients, families, and multidisciplinary teams, fostering collaboration and advocacy.
