Juvenile Justice Case Manager - LYNC Program
Location: Greater Hartford Organization: The Village for Families & Children Employment Type: Full-Time
Salary: $48,000-$50,000 commensurate with experience. It is not typical for an individual to be hired at or near the top of the range for their role. Actual placement within range will be contingent upon a number of factors, including but not limited to the candidate's qualifications, education, experience, internal equity and alignment with market data.

Job Summary
The Juvenile Justice Case Manager position is responsible for care coordination through provision of a range of case management and support services to LYNC youth and families, in coordination with other LYNC staff, agency, and community providers. Works closely with LYNC Service Specialists in identifying and connecting youth and families to community resources
KEY RESPONSIBILITIES
1. Provision of direct services to youth and their families through care coordination which will include:
a. Conducts clinical assessment and engagement;
b. Develops a Risk Reduction Sustainability Plan (RRSP);
c. Implements and monitors RRSP with youth and family, modifying as appropriate during the course of service;
d. Structured case management, using Carey Guides;
e. Discharge planning including plans for follow up treatment and connection to community services and programs;
f. Works with community organizations to connect youth and families to services.
2. Participates and collaborates in all phases of LYNC: Engagement, Service, and Sustainability, as part of the LYNC team to provide services to referred youth and families.
3. Engages in strength-based, culturally competent, trauma-informed and gender responsive interactions with youth and families.
4. Upholds all program expectations.
5. Promotes a safe, welcoming environment with a focus on risk reduction and positive behavior change.
6. Acts as point of contact for youth and families during service delivery.
7. Accurate and timely completion of documentation and reports, and any other documentation and reports required by agency and program policies and funding sources.
8. Develops and maintains collaborative relationships with other community agencies and organizations in consultation with supervisor, in order to provide comprehensive and effective treatment services for clients (i.e. schools, courts, community organizations, etc.)
9. Works as a team member to respond to client and programmatic needs.
10. Assists in providing transportation to LYNC youth and families as needed.
11. Attends all trainings and meetings as required for the position.
12. Accepts and performs all new assignments as determined by supervisor.
13. Notifies supervisor immediately of any situation in which a youth may be at risk (i.e. physical abuse, sexual abuse, chronic neglect, un-safe home, etc.) and/or other sentinel event.
14. Knowledge and implementation of program and agency policies and procedures.
15. Partners with supervisor to identify areas and develop plans for professional growth.
16. Contributes to agency organizational life by participating in agency wide task forces, committees or problem solving teams as appropriate.
17. Contributes to agency visibility and development of community linkages by representing the Village in community groups or committees, as assigned by supervisor.
18. Other duties or responsibilities as determined by management.
QualificationsEDUCATION
· Bachelors Degree in Criminal Justice, Sociology, or Human Services OR a High School Diploma with a minimum of three (3) years of experience working with justice/system/agency involved youth.
EXPERIENCE
· Minimum of three years proven experience working with justice/system/agency involved youth.
· Excellent oral and written skills and ability to communicate comfortably with children, parents, and school personnel.
· Strong case management skills.
· Effective engagement skills.
· Strong community connections and knowledge base.
· Organized and able to work independently with minimal supervision.
· Flexible and able to multi-task.
· Cultural competent to serve targeted populations.
· Bicultural, Bilingual skills preferred.
· Experience working with youth involved in the juvenile justice system.
· Demonstrated understanding of risk-needs-responsivity principles.
· Must have current valid state issued Driver License.
· Knowledge of greater Hartford and New Britain area community resources.
· Commitment to Results Based Accountability.
· Proficient with word processing and spreadsheet applications. Possess solid general computer skills, including ability to work in a MS Office environment. Familiarity with specific applications used by the Village a plus.
· May require weekend and evening hours.
· Able to function safely in therapeutic situations.
· Able to travel to various sites throughout greater Hartford and New Britain.
