What You'll Do

• On-Site Coordination: Work directly within the hospital to facilitate quality, cost-effective care transitions for renal patients.
• Reduce Fragmentation: Act as a dedicated advocate to ensure continuity of care between the hospital, the family, and the outpatient provider.
• Performance Coaching: Apply CQI techniques and clinical data to drive better patient satisfaction and health outcomes.

What You Bring (The "Shining Star" Profile)

• Licensure: Current, good-standing CA Registered Nurse (RN) license.
• Experience: Minimum of 3 years in clinical nursing (Required).
• Leadership: Background in healthcare performance coaching or acting as a "change agent."
• Preferred: Renal nursing experience, BSN, or certifications like CNN or CCM.
• Logistics: Valid CA driver's license with a positive record.
