Community Bridges, Inc. is seeking a Specialty Case Manager to provide in-community case management for individuals with mental health or substance use diagnoses, survivors of domestic violence, and high‑risk clients. You will plan, connect, advocate, coordinate, and monitor services to access health care, housing, employment, education, and supports, while documenting accurately.
