Registered Nurse (RN) Weekend

About the Role We are a post‑acute care provider committed to the growth and development of our team and dedicated to caring for patients like family. As a Registered Nurse you will provide one‑on‑one patient care in home settings, working at the top of your license within an interdisciplinary team. Schedule & Territory Schedule: 2 weekdays and 1 weekend day per week, 8am to 5pm, plus one on‑call shift per month and holiday on‑call rotation. Territory includes Hamilton, Oxford, Trenton, Fairfield, Colerain, Blue Ash, Lockland, West Chester, Miamitown, Queen City, Hyde Park. Responsibilities Plan of Care: Observe and monitor patient conditions, perform OASIS assessments, develop and adjust individualized care plans. Minimize Patient Risk: Administer medication as prescribed, prioritize visits for high‑risk patients to reduce rehospitalizations. Continuity of Care: Manage multidisciplinary care and promote continuity through admissions, transfers and discharges. Family Educator/Advocate: Educate patients and families on disease processes using teach‑back methods. Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Rules & Regulations: Follow agency policies, procedures, rules, regulations and report schedule changes. Operations: Attend in‑service training and mandatory agency meetings. Hard & Soft Skills Compassionate communicator with a positive attitude. Patience when working with patients, families, physicians and coworkers. Attention to detail and observant of instructions. Requirements Registered Nurse with a current license in the state of employment. Minimum one year of RN experience in an acute‑care setting. Home‑care experience preferred. Valid driver’s license and auto insurance in your name as a driver. Capable of meeting all physical demands of the role. Benefits Guaranteed base salary plus an uncapped bonus structure.
