Description

1. PRIMARY PURPOSE OF POSITION:

The Health Information Clerk plays a crucial role in the smooth operation of the nursing unit by providing administrative support to the nursing staff and ensuring effective communication within the unit.

II. CUSTOMERS/CLIENTS SERVED BY POSITION:

Board members Staff Residents family members guests vendors and other outside clients served by Ovation Communities.

III. Qualifications:

• High School diploma or equivalent.
• At least one year pf clerical experience in a health care setting. Experience in a senior living center preferred.
• Experience as a Certified Nursing Assistant (CNA) preferred.
• Must be able to read write and speak the English language

IV. JOB DUTIES:

1. Performs clerical duties such as answering phones managing patient records scheduling resident appointments and transportation and handling correspondence.
2. Serves as a liaison between nursing staff patients families and other healthcare professionals to facilitate effective communication.
3. Greet and assist visitors ensuring they follow facility protocols and providing them with necessary information.
4. Prepares documents for emergency resident transfer needs.
5. Assists with the admission process by preparing necessary paperwork verifying insurance information and orienting new patients to the unit.
6. Maintains accurate and up-to-date patient records including admission and discharge information medical histories and treatment plans. Accurately scans/enters resident information into electronic health record (EHR) and other databases.
7. Assembles discharged medical records in a timely and accurate manner and deliver to Medical Records department.
8. Monitors and orders office supplies and medical equipment as needed to ensure the unit is well stocked.
9. Ensures compliance with all regulatory requirements and facility policies and procedures.
10. Provides excellent customer service to patients families and visitors addressing their needs and concerns promptly and professionally.
11. Promptly notifies Nurse Manager/designee of all concerns grievances complaints.
12. Maintains and promotes all professional aspects of HIPAA.
13. Communicates with Nurse Manager throughout shift to coordinate efforts and to meet resident and staff needs.
14. Uses personal protective equipment when indicated by exposure control plan; comply with all infection control procedures.
15. Attends in-service education programs as scheduled.
16. Performs other duties as assigned.

2. Working knowledge of the facility record keeping policies and procedures and ability apply knowledge to job tasks.
3. Sufficient interpersonal skills and maturity to communicate effectively with professional staff direct care staff physicians consultants residents visitors and others in person and on the telephone.
4. Ability to establish approximate priorities for receiving recording relaying and acting on information and to maintain organization on all unit clinical and administrative records.
