Requirements ***Client has 8hrs of Non-Billable hours for orientation hours *** At time of Offer please have provider's DOB SSN ready to submit*** as this is required to present to client within 24hrs of an offer. *** Include 4hrs of gratis for all call Da Vinci experience is a huge plus but not required. Start- ASAP - Washington state license must be in hand at time of present. No IMLC at this time. We are going to seek fast track or urgent credentialing. Type- MD/DO • Yearly refills • Diagnose and treat erectile dysfunction • Kidney stones • Male incontinence • Cystoscopies • Surgical assist Why Open?- Gap in coverage Working at Medical Center and Urology Clinic 1 mile away Both Hospital and Clinic Privileges Needed Inpatient Outpatient Setting Bed #- 122 Call 7 days of unrestricted call per month Avg. Patients per day- 18 Procedures- standard urology procedures in the clinic and operating room Required clinical activity in the last 24 months case logs- Applicant must provide documentation case list of provision of surgical services 100 procedures representative of the scope and complexity of the privileges requested during the previous 24 months. Case list must include diagnosis or procedure and date and facility where performed. Will provider work solo? If not other providers specialties working with them- Yes Licensed Candidate: now open to seeing candidates with an IMLC but they must have the letter of qualification in hand as well. It is our understanding that with an LOQ a Washington license can be issued in less than week for MD and about 3.5 weeks for DO. Required certifications Must be from American Heart Association or American Red Cross as applicable- BLS and ACLS EMR - EPIC Procedures being Performed: Catheter placement STENT Placement Cystoscopy TURP Ureteroscopy prostate biopsy Circumcision Shock wave lithotripsy ESWL Orchiopexy turbt hydrocelectomy spermatocelectomy PVP litholapaxy ***Admin Time is paid at an hourly rate *** Call is restricted/Beeper with a 10' return call response time a radius of 30 mins might be 45 ***Average After hour text/call 0-4 Shift Info: Day 5x8-Hour 08:00 - 17:00
