Willingness to travel throughout this region is required.While Findlay/Fostoria/Tiffin are a centralized "home base" to cover this region with minimal travel, Qualified Applicants may reside in or near any part of the service areas so long as travel requirements are fulfilled.This is a weekday position (Monday - Friday) requiring primarily daytime availability (8am -5pm) with occasional evening availability.No Weekends required. Job SummaryAs an RN Recertification Nurse, you will be instrumental in coordinating, re-evaluating and confirming the medical necessity of continued home health care services for our patients, and providing patient care and ensuring optimal outcomes.With core skills in managed care, patient assessments and compliance, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.You will submit updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines.You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.
