HCS Program Support Coordinator - Intellectual Developmental Disabilities

Tropical Texas Behavioral Health

Description

Duties include but are not limited to coordinating the delivery of each individual's Individual Plan of Care
(IPC), monitoring the delivery of waiver and generic services, integrating the various aspects delivered under the waiver, and recording each individual's progress. This would include attending team meetings as appropriate, writing implementation plans, IPC's, ID-RC's, ICAP's and justifying waiver services. Case
Manager works under supervision of the Program Administrator with some latitude in the use of initiative and independent judgment.

Requirements

Bachelor's degree from an accredited college or university with a major in social work, psychology, or a field related to human behavior, plus one (1) years' experience working with individuals with developmental disabilities. . Bilingual (English/Spanish) required.
