Care Coordinator Position at Lifekind HealthLifekind Health is looking for a Care Coordinator to join our team in Victorville. Ensure that patients receive comprehensive and continuous healthcare services tailored to their individual needs. Act as a liaison to facilitate communication among multidisciplinary teams, ensuring that care delivery is seamless and efficient. Clinics are open 7 days a week, 8:00 am - 7:00 pm. Availability to work Saturday and/or Sunday is required.About the position:Assess patient needs and develop individualized care plans in collaboration with healthcare professionals and patientsCoordinate appointments, treatments, and follow-up care to ensure continuity and adherence to care plansServe as the primary point of contact for patients and families, providing education and support throughout the care processFacilitate communication between patients, healthcare providers, and community resources to address social determinants of healthMonitor patient progress and update care plans as necessary, documenting all interactions and outcomes accuratelyIdentify and address potential barriers to care, including transportation, financial constraints, and language differencesMaintain compliance with healthcare regulations, privacy laws, and organizational policiesParticipate in interdisciplinary team meetings to discuss patient care strategies and improve service deliveryOur Ideal Candidate will have the following qualifications:At least 2 years of experience in care coordination, case management, or a related healthcare roleCertification in Medical Assisting (MA), Case Management (CCM) or Certified Care Coordinator (CCC) credential a strong plusHigh School Diploma or equivalent requiredStrong knowledge of healthcare systems, patient care processes, and community resourcesExcellent communication and interpersonal skills to effectively interact with diverse patient populations and healthcare teamsProficiency in electronic health records (EHR) and basic computer applicationsExperience working with vulnerable populations, including elderly or chronically ill patients preferredBasic Life Support (BLS) or CPR certification is a plusBilingual abilities to support non-English speaking patients preferredAt Lifekind, we believe our employees deserve the best.
