HSS Service Coordinator (LVN) – Southeast Harris County, TX – 2347637 The HSS Service Coordinator will be an essential member of the STAR Kids care team, working within an integrated care model to relay member needs, advocate for the best possible care, and ensure the right services are provided. This hybrid position requires up to 75% of the time in the field and up to 25% from a home‑based office. Telecommuters are welcome if you reside in Southeast Harris County, TX (Pasadena, Deer Park, and surrounding areas). All telecommuters must adhere to UnitedHealth Group’s Telecommuter Policy. Primary Responsibilities Assess, plan, and implement individualized care strategies focused on the most appropriate, least restrictive level of care. Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services. Manage the care plan throughout the continuum of care as a single point of contact. Communicate required health‑related information to all stakeholders to ensure coordinated, quality care is delivered promptly. Advocate for patients and families to ensure their needs and choices are fully represented and supported by the health care team. Visit members in their homes or other settings such as community centers, hospitals, nursing facilities, or providers’ offices. The hourly pay range for this role is $23.89 to $42.69 per hour, based on full‑time employment, adjusted for local labor markets and experience. Required Qualifications Current and unrestricted LVN/LPN license in Texas. 2+ years of experience in community health or a healthcare setting. Intermediate proficiency in Microsoft Word, Excel, and Outlook. Reliable transportation and ability to travel in assigned region. High‑speed internet access at residence. Valid US driver’s license. Preferred Qualifications 2+ years of pediatric care experience. Experience in pediatric case management and/or certification in case management (CCM). Field‑based work experience. Electronic charting experience. Background in managing populations with behavioral needs.
