Requirements:

• Active state license, certification, or credential, as required for your discipline
• Ability to meet all agency, state, and federal compliance requirements
• Prior home health experience preferred, but not required for all roles

Qualifications:
Qualified applicants MUST have at least 2 years of experience in home health or community care.
Qualified applicants MUST have a valid healthcare or community care credential/certificate.
Experience with OASIS and related home health documentation tools is a strong asset.
Preferred Skills:

• Hospice care experience

• Behavioral health & counseling skills

• CNA support experience

• Valid Maryland CNA license, or applicable license for your discipline

• Willing to obtain necessary licensure if moving to different states

As a healthcare professional in a home health or community-based care setting, you will:

• Provide care to patients in their homes or community environments in accordance with care plans and scope of practice.
• Assess, monitor, and support patient needs in a home-based environment.
• Maintain accurate documentation in compliance with regulatory and agency standards.
• Collaborate with interdisciplinary care teams to support patient outcomes.
• Educate patients and caregivers on treatment plans and ongoing care.
