Discharge & Care Coordination Social Worker

Howard University Hospital in Washington, DC is seeking a compassionate Social Worker to join our Case Management team. You will assess psychosocial needs, coordinate discharge planning, and connect patients and families with post-acute services. The role requires DC licensure (LGSW/LISW/LICSW), an MSW, strong communication, crisis intervention skills, and the ability to work with a multidisciplinary team. Evening, weekend, and holiday coverage may be required; compensation ranges hourly under #J-18808-Ljbffr
