About the Position This position is primarily responsible for providing a full range of clinical social work services to residents of The View Alexandria and their families to promote general social and emotional well‑being and to identify their social and emotional needs which may be related to medical conditions. Pay: Between $82k and $94k Key Duties Interview potential residents to complete a social history and cognitive assessment to determine level of care. Provide follow‑up Social Work visits to newly admitted residents and document such visits within ninety (90) days after arrival in the resident’s medical record. Offer direct social work interventions to residents and their families, providing appropriate measures for helping residents with personal adjustment, emotional and social well‑being and accessing facility and community referrals and resources. Document in the clinical chart appropriate conversations, meetings, and interventions with residents, families/POA, and staff concerning residents as required by Federal and State regulations and TVA policy. Administer the Mini‑Mental Status Exam, Montreal Cognitive Assessment, Geriatric Depression Scale, Brief Cognitive Rating Scale, or other comparable assessments as needed. Achieve certification by the Virginia Department of Social Services (VDSS) to participate in the completion of the Universal Assessment Instrument (UAI) and development of the Individualized Service Plan (ISP) within 90 days of hire; maintain all associated records and document service plan meetings with the resident, interdisciplinary team and family/powers of attorney annually or when significant change occurs. Coordinate services to residents of on‑site nursing home with medical and nursing staff to include care plans, MDS, and documentation as required by the Virginia Department of Health. Collaborate with the interdisciplinary team to complete the Minimum Data Set (MDS) within 14 days of resident admissions, quarterly, annually, and as required by change in resident status. Counsel residents and families to discuss and understand feelings, attitudes, fears, and anxieties related to problem solving and explore alternative solutions. Coordinate clinical social work services with the hospice social worker for residents who are hospice patients. Develop and maintain contact with appropriate community agencies and services to promote cooperation and facilitate referrals. Participate in the development and implementation of resident care policies related to psycho‑social aspects of resident care issues. Participate in the weekly interdisciplinary team meeting and collaborate to assist with admission, transfer and discharge planning; provide follow‑up and complete required forms (VDSS Discharge Notification, Assisted Living Transfer Form, social services notes in EHR, etc.). Provide quality resident care by coordinating conferences with families/powers of attorney and other interested parties to address adjustment, treatment, and caregiving concerns. Advocate for resident rights in cooperation with other facility personnel, attend resident council meetings and coordinate a yearly resident rights update for all residents. Educate on and promote the completion of Advanced Directives, as required. Provide assistance and information to residents and families/POAs related to special issues (e.g., US Census, annual TVA tax information letter, voting information, etc.). Develop and present in‑service training as required. Provide Social Work orientation to interns and new staff members within the department, as necessary. Provide support groups for residents, family members, and the local community when appropriate. Job Requirements BSW or MSW degree from an accredited university; preference for a licensed clinical social worker (LCSW). Must possess computer skills; preference for experience with electronic medical records. Prefer at least three (3) years experience in social work, with at least one (1) year working with the geriatric population. Must possess ability to deal tactfully and effectively with residents, staff, family members and the public.
