Physical Therapist

Vantage Health Partners is seeking a Physical Therapist to join our Home Health/Assisted Living Facilities team covering Loveland/Fort Collins, CO. This remote position within the Hospital/Health Care industry offers the opportunity to deliver high-quality rehabilitative care and help patients regain mobility, strength, and independence. This full or part time, remote based position offers an opportunity to make a meaningful difference in patients' lives by helping them regain independence and improve their quality of daily living within the Hospital/Health Care industry. As a HH/ALF Physical Therapist, you will evaluate patients, develop individualized treatment plans, and guide them through therapeutic exercises and interventions. You will work closely with physicians, nurses, and other healthcare professionals to ensure coordinated, patient-centered care that supports long-term recovery and wellness. This clinical role provides a supportive environment for professional development and clinical growth. Compensation is offered on a pay per visit or salaried option, with details shared during the hiring process. Responsibilities Evaluate patients' physical condition, mobility, and functional limitations Develop and implement individualized physical therapy treatment plans Perform therapeutic exercises, manual therapy, and rehabilitation techniques Educate patients and families on injury prevention and home exercise programs Monitor and document patient progress and treatment outcomes accurately Adjust treatment plans based on patient response and clinical findings Collaborate with physicians and interdisciplinary care teams Maintain compliance with clinical protocols, safety standards, and regulatory requirements Provide clear communication and compassionate care to patients and their families Participate in continuing education to stay current with best practices in physical therapy Requirements Doctor of Physical Therapy (DPT) degree from an accredited program Current state licensure as a Physical Therapist in Colorado or eligibility to obtain licensure Strong knowledge of musculoskeletal, neurological, and cardiopulmonary rehabilitation Proficiency in patient assessment, documentation, and clinical reasoning Excellent interpersonal and communication skills Ability to work effectively in a fast-paced, on-site healthcare environment Strong commitment to patient safety, empathy, and quality of care Preferred: Experience with electronic medical records and outpatient or hospital-based rehabilitation Compensation & Benefits Supportive work environment with opportunity to learn and develop Pay per visit or salaried based option Mileage reimbursement Competitive benefits package 401K plan + 15% matching Company phone provided PTO given up front (FTE) Opportunity to provide individualized care to patients Continuing education opportunities Tablet Purchase Program #J-18808-Ljbffr Vantage Health Partners
