Job Title: RN: Case Manager
Location: Lexington, KY
Setting: Home Health
Pay Range: $49.07 - $51.52 per hour
Schedule: 40 hours per week, Monday through Friday, 8am-5pm (On call required; 1 weekend out of the month, 1 day each week)

Requirements:

• At least 2 years of experience as a RN (Registered Nurse)
• Qualified applicants MUST have current, unrestricted Kentucky or compact nurse license
• Qualified applicants MUST have Home Health experience, OASIS, and HCHB
• Ability to analyze complex medical information and make rational decisions
• Case Management certification
• Be willing to obtain State licensure

Responsibilities:
As a healthcare professional in a home health setting, your role will include:

• Performing comprehensive assessments of high-risk members, analyzing complex medical data, and developing individualized care plans.
• Collaborating with primary care providers and other healthcare professionals to implement targeted interventions that engage members and promote health outcomes.
• Addressing, identifying, and reassessing cost-effective, appropriate levels of care, and putting non-medical support services in place to ensure compliance with treatment plans, including housing and transportation.
• Coordinating care transitions among physicians, social workers, discharge planners, and claims professionals, and engaging specialty resources as needed to achieve optimal patient outcomes.
• Keeping detailed records of clinical, functional, and fiscal outcomes, and identifying opportunities for health promotion and illness prevention.
• Acting as a patient advocate by protecting privacy and confidentiality issues and intervening quickly to minimize poor outcomes.
