Job Summary:

Brookside Care Center, a leading healthcare provider in Stockton, CA, is seeking a motivated and compassionate Physical Therapy Assistant to join our interdisciplinary team. As a Physical Therapy Assistant, you will play a vital role in delivering high-quality patient care and contributing to the recovery and well-being of our residents.

Responsibilities:

• Assist physical therapists in planning, implementing, and evaluating treatment plans for patients with a range of medical conditions and injuries

• Conduct physical therapy treatments, including exercises, stretches, and modalities, under the supervision of a licensed physical therapist

• Monitor patient progress and report changes to the physical therapy team

• Communicate effectively with patients, families, and healthcare staff to ensure seamless care transitions

• Maintain accurate and thorough records of patient treatments and progress

• Participate in patient education and family training programs to promote independence and successful discharge

Requirements:

• Graduate of an accredited Physical Therapy Assistant program

• Possess a current Physical Therapy Assistant license to practice in the state of California

• A strong desire to learn and grow in a dynamic healthcare environment. New grads welcomed!

• Ability to lift, transfer, and position patients with varying abilities and needs

• Excellent communication, teamwork, and problem-solving skills
