Impactful Housing Stability Case Manager
Lifting Up Westchester
Lifting Up Westchester is seeking a dedicated Case Manager to support our Housing team in White Plains, NY. You will manage a primary caseload of chronically homeless individuals eligible for rental assistance, with regular home visits and calls to connect clients with housing stabilization services. Under supervision, you will conduct intakes, assist with services, coordinate with community resources, document case notes, and help clients meet housing goals. #J-18808-Ljbffr
