Intake Worker

Description As a regional agency of Catholic Charities of the Diocese of Rochester, Catholic Charities Family and Community Services (CCFCS) has been serving our community for over 100 years. We are looking for an Intake and Placement Manager to join our team in Rochester, NY. This full‑time position offers a dynamic, impactful role serving the community daily. Essential Duties and Responsibilities Develop, implement, and monitor policies and procedures for client outreach, engagement, and centralized intake processes. Serve as an active member of the management team in planning, problem solving and ongoing communication. Provide supervision and guidance to intake and outreach/engagement staff, including: Regularly meeting with direct reports. Completing annual performance appraisals. Monitoring performance and providing appropriate feedback. Participate in hiring, onboarding, retention, coaching, progressive discipline, and separation activities in coordination with Human Resources. Assist in budget development and monitor budgeted expenses for outreach and intake activities. Facilitate a variety of administrative projects relevant to outreach, engagement and intake, ensuring projects are within scope, timeline, and budget limits. Assist in the design of outreach and engagement materials, marketing and placement plans, in coordination with marketing and program development, tailoring messaging to clients, families, providers, and community partners. Lead efforts for an agency‑wide data system for client demographics, service inventory, and Social Determinants of Health in coordination with IT and data system staff. Serve as lead to implement and fully utilize referral systems in coordination with IT and other community partners. Develop, implement, monitor, and evaluate the agency outreach and engagement plan. Coordinate and schedule staff for agency and program‑related outreach, engagement at community events, fairs, and network opportunities. Serve as a role model for staff and demonstrate CCFCS values in outreach, engagement, and intake activities. Ensure outreach, engagement, and intake utilize best practices and trauma‑informed approaches in coordination with peer services. Develop internal information networks to improve staff education and knowledge of the CCFCS spectrum of services. Represent CCFCS on community boards, networks, and collaborations, and provide communication and education on CCFCS services and referral processes. Actively pursue development of professional competencies related to job performance through reading, supervision, in‑service training and attendance at conferences and workshops. Participate in relevant agency meetings and trainings. Support and foster the agency's mission, vision, values, and strategic plan. Other duties as assigned. Requirements Qualifications Education: Bachelor’s Degree in health and human services or related field. Equivalent combination of education and experience will be considered. Credentials: Valid and clean NYS Driver License. Experience: Three years' experience with intake or assessment activities in health and human services, and three years of supervisory experience.
