You will start on a PRN basis, picking up shifts based on your availability and patient demand with a genuine pathway to a more consistent, long‑term role as volume increases and service footprint expands. In this role, you will conduct clinical assessments enabling patients to access Community Health Worker (CHW) services, helping them overcome barriers to care, social services, improve stability and achieve better health outcomes through appropriate referrals and utilization of medical and social services. This is a patient‑centered role that blends clinical judgment with care coordination in a value‑based, whole‑person care model. Why This Role Matters For many patients, health outcomes are shaped as much by housing instability, transportation challenges, and access to resources as determined by clinical conditions. This role plays a critical part in identifying those barriers and activating the right supports. As a Nurse Practitioner on this team, you serve as the clinical gateway to CHW services, helping to improve patient experience, reduce avoidable emergency department use, improve continuity of care, and ensure patients receive the right support at the right time. What You'll Do Conduct telehealth clinical evaluations to assess acute and chronic health needs, preventive care needs, and social drivers of health. Determine clinical appropriateness for CHW services and make referrals for care coordination and community-based support when indicated. Develop individualized care plans addressing both clinical needs and social determinants of health factors. Collaborate closely with Community Health Workers to support patient engagement and follow-through. Address appropriate acute, chronic, preventive, and medication-management needs within the scope of the program, including prescribing and renewing non-controlled medications when clinically indicated. Document visits accurately and timely in accordance with clinical, state, federal, and program requirements. Help patients navigate complex healthcare systems and connect to appropriate medical and community resources. Qualifications Active, unrestricted Nurse Practitioner/APRN license and all applicable prescribing or furnishing authority required for practice in the state(s) in which the NP will provide care. Must be licensed in at least one of the following states: Alaska (AK) Arizona (AZ) California (CA) Louisiana (LA) Michigan (MI) New Mexico (NM) Nevada (NV Licensure in multiple states strongly preferred, with willingness to obtain additional state licenses as needed. Experience in primary care, family medicine, internal medicine, urgent care, or chronic disease management strongly preferred; telehealth experience a plus. Experience working with Medicaid and Medicare populations. Strong understanding of social determinants of health and their impact on patient outcomes. Comfortable with telehealth platforms and building rapport virtually. Excellent communication skills and ability to engage patients from diverse backgrounds. Familiarity with state, federal, and payer requirements applicable to CHW services and referrals, or willingness to learn. Spanish or other language fluency strongly preferred Projected Time Commitment This position starts as true PRN. There is no guaranteed minimum number of hours per week. Shifts are offered based on patient demand, and you select which you are available for. We will work with you to build a schedule that fits your life. Phase 1: PRN For the next 3-6 months Flexible scheduling; no minimum hours guarantee; competitive hourly rate Phase 2: Regular Part-Time As patient volume scales (est. 6–12 months) Consistent weekly schedule offered; 10+ hrs/week; priority scheduling access Phase 3: Full-Time As we reach regional/national scale Full-time salaried or high-hour contract role; benefits eligibility where applicable Compensation The hourly rate for this role starts at $80.00. Compensation Range Disclosure Pear Suite is a fully remote organization so compensation may be adjusted upward for candidates residing in higher-cost labor markets.
