Job Title: Charge Nurse

Shift : 2PM - 10PM

As a Charge Nurse at San Antonio Wellness and Rehabilitation Center, you will play a pivotal role in leading our skilled nursing and rehabilitation team. Responsibilities:

• Lead the nursing team : Supervise and coordinate the daily activities of the nursing staff, ensuring that patients receive high-quality care and services.

• Plan and implement care : Develop and implement individualized care plans, collaborating with physicians, therapists, and other healthcare professionals to achieve optimal patient outcomes.

• Monitor and evaluate : Continuously monitor patient progress, identifying areas for improvement and implementing changes to enhance the quality of care.

• Collaborate with interdisciplinary team : Work closely with physical therapists, occupational therapists, speech therapists, and other healthcare professionals to ensure that patients receive comprehensive and coordinated care.

• Maintain accurate records : Accurately document patient information, progress notes, and other relevant data to ensure compliance with regulatory requirements and internal policies.

Requirements:

• Current LVN or RN licensure : Must be a licensed Registered Nurse in the State of Texas.

• At least 2 years of experience :

• BLS certification : Must possess Basic Life Support (BLS) certification.

• Strong communication and leadership skills : Excellent interpersonal and communication skills, with the ability to lead and motivate a team.

• Adaptable and flexible : Ability to work in a fast-paced environment, with a focus on quality patient care and continuous improvement.
