Key Responsibilities:

• Provide personal care services, including bathing, grooming, dressing, toileting, and skin care.
• Provide touch therapy to patients as indicated in the plan of care when appropriately certified and trained.
• Assist patients with mobility, ambulation, positioning, transfers, and prescribed exercise programs.
• Support patients with self-administered medications as directed in the plan of care.
• Assist with meal preparation, nutrition, and hydration needs.
• Perform light housekeeping tasks that are essential to maintaining a safe and healthy home environment.
• Promote patient independence, dignity, comfort, and quality of life.
• Observe, document, and promptly report changes in patient condition, safety concerns, or unmet needs.
• Communicate effectively with patients, families, supervisors, and members of the interdisciplinary team.
• Complete required documentation accurately and according to agency standards and timelines.

• Successful completion of a state-approved Licensed Nursing Assistant (LNA) training program;
• Current New Hampshire LNA license in good standing;
• Current CPR certification;
• Successful completion of agency competency requirements;
• Reiki certification or willingness to obtain certification within six (6) months of employment;
• Valid driver’s license, reliable transportation, and required automobile insurance; and
• Ability to complete a minimum of twelve (12) hours of annual in-service education.

Preferred Qualifications:

• One year of experience as an LNA;
• Experience in home health, hospice, long-term care, or hospital settings; and
• Hospice or end-of-life care training.
