Physical Therapist
MEP Health
Role: Licensed Physical Therapist providing personalized geriatric care in private residences and senior living communities. Responsibilities Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy to older adults. Support patient mobility, independence, and wellness through one-on-one sessions. Document patient progress accurately in electronic medical records. Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff on safety, function, and aging-in-place strategies. Incorporate telehealth and virtual care to enhance access and flexibility. Qualifications & Skills Valid Physical Therapy license and degree from an accredited program. Proficiency in computer use and EMR systems. Current CPR certification. Ability to meet physical demands including lifting, repositioning patients, and performing therapeutic activities. Key Attributes: Autonomous, flexible, committed to meaningful patient outcomes, and skilled in building strong patient relationships. Experience in in-home and senior living settings is preferred.
