Position Overview

We are seeking a passionate, tech-savvy, and outreach-focused Enrollment Representative to help seniors and their caregivers navigate the enrollment process for comprehensive, community-based senior healthcare services.

This role serves as a primary point of contact for prospective participants and their families , guiding them from initial inquiry and qualification through documentation, Medicaid enrollment, approval, and onboarding. The ideal candidate will have healthcare sales, admissions, intake, managed care, senior services, or insurance experience and be comfortable working directly with both individuals and professional referral sources.

This is an opportunity for a relationship-driven self-starter who wants to help seniors maintain independence, age in place, and improve their quality of life.

Key Responsibilities

Potential Participant Support - 40%

• Receive and follow up on qualified referrals from call centers, brokers, healthcare professionals, and community partners.
• Schedule and conduct intake appointments, home visits, center visits, and tours.
• Gather and accurately document participant information and service needs in the CRM system.
• Educate prospective participants, families, and representatives about available healthcare services.
• Build trust and provide exceptional customer service throughout the enrollment experience.
• Identify appropriate services based on each individual's healthcare and personal needs.
• Use effective communication and sales techniques to move qualified prospects toward enrollment.
• Identify opportunities for additional referrals through participant and referral-source relationships.

Enrollment Process Management - 50%

• Serve as the primary point of contact throughout the qualification and enrollment process.
• Maintain accurate, real-time updates regarding prospect status in the CRM.
• Collect required documentation and information from applicants and their representatives.
• Coordinate and monitor the Medicaid application and eligibility process.
• Ensure applications and enrollment documentation are completed accurately and within required timelines.
• Present prospective participant cases to internal enrollment teams and advocate for appropriate enrollment.
• Collaborate with eligibility, financial determination, Medicaid, and other departments to resolve enrollment issues.
• Use strong sales and closing skills to successfully convert qualified prospects into enrolled participants.
• Conduct home and day-center visits as needed to complete enrollment requirements.

Post-Enrollment Support - 10%

• Coordinate with interdisciplinary teams and internal departments to ensure a smooth transition after enrollment.
• Provide appropriate handoffs to operational and clinical staff.
• Support community outreach and efforts to strengthen awareness of senior healthcare services.
• Maintain positive relationships with participants, families, referral sources, and community partners.
• Perform additional duties as needed to support the enrollment team.

Qualifications

• 2+ years of successful healthcare-focused sales, intake, admissions, or enrollment experience preferred.
• Experience working with both B2B referral sources and B2C patients or families .
• Background in healthcare, insurance, senior care, managed care, Medicare Advantage, Medicaid, Veterans Administration benefits, or a related field.
• Proven ability to close sales and convert qualified prospects into enrollments .
• Strong knowledge and experience using CRM systems , such as Salesforce, Microsoft Dynamics, HubSpot, Zoho, or similar platforms.
• Bachelor's degree in business, sales, marketing, social work, or a related field preferred; equivalent relevant experience may be considered.
• Excellent communication, presentation, organization, and relationship-building skills.
• Strong customer service and active listening abilities.
• Ability to manage multiple cases and deadlines while maintaining accurate documentation.
• Comfortable conducting home visits and working in a field-based environment.
• Valid driver's license, reliable personal transportation, and required auto insurance.
• Good driving record.
