**Job Title: Care Manager (CNA/HHA) - Part-time / PRN (2 PM - 10 PM)**

We are seeking a dedicated and compassionate Care Manager (CNA/HHA) to join our team. This position offers flexibility with part-time and PRN (as needed) shifts from 2 PM to 10 PM, providing you with an excellent opportunity to make a meaningful impact on the lives of our clients while balancing your personal schedule.

**Responsibilities:**

- Provide high-quality personal care and companionship for clients in a safe and respectful manner.
- Assist clients with daily activities, including bathing, dressing, grooming, and meal preparation.
- Monitor and record patients’ medical history, vital signs, and overall health condition accurately.
- Collaborate with healthcare professionals to plan and evaluate personalized care plans.
- Maintain a clean and orderly environment to ensure the safety and comfort of clients.
- Assist with mobility and physical exercises as prescribed by healthcare practitioners.
- Respond to emergencies promptly and effectively, ensuring the welfare of the client.
- Offer emotional support to clients and their families by maintaining open and positive communication.
- Uphold all company policies and procedures in accordance with healthcare regulations.

**Qualifications:**

- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification is required.
- Proven experience in a caregiving position is highly preferred.
- Ability to handle multiple tasks efficiently while maintaining exceptional attention to detail.
- Excellent interpersonal and communication skills.
- Compassionate, patient, and committed to providing the best possible care for clients.
- Ability to work independently while also collaborating effectively with a healthcare team.
- Flexibility to work the hours as specified, including weekends and holidays as needed.
