Intensive Case Manager - Inpatient

Responsibilities Provide intensive case management for a targeted group of high‑risk mental health clients in a local community. Act as a single identifiable focus for service responsibility and function as the primary support in a comprehensive and individualized approach to integrating clients into the community. Ensure clients’ basic living needs are being met and advocate for and assist clients in gaining access to and maintaining medical, psychiatric, self‑help, rehabilitation, social, educational, psychological, and other necessary support services. Intensely monitor clients’ integration progress and overall service delivery. Respond directly and immediately to clients and other involved parties on‑site (emergency rooms, residence, other locations) in order to determine and address immediate needs for housing, respite, intervention, and support services. Receive and respond to referrals and/or requests for assistance from community‑based agencies and organizations. Conferring with clients, professional medical/clinical personnel, providers, and other concerned parties to identify unmet needs and necessary supports/services and develop a care plan based on clients’ needs, preferences, circumstances, and team assessments. Assist clients in accessing and maintaining needed services to support community integration. Maintain intensive personal contact with clients and concerned parties to determine clients’ ongoing psycho‑social and community support needs and monitor the effectiveness of treatment plans and service delivery. Ensure clients’ immediate psychiatric, social, and situational needs are met through medical intervention, basic living necessities, respite, and other support services to minimize crises, prevent hospitalization, and promote community integration. Record and document contacts with clients and service providers to document case history for planning and accountability purposes. Conferring with families, providers, medical/clinical personnel, and other concerned parties to exchange information concerning clients’ progress and needs. Participate in inpatient treatment and discharge planning to ensure community of care and brief and appropriate hospital stays. Identify, advocate for, and obtain necessary support and services for clients to provide timely interventions, address unmet needs, and reduce the risk of hospitalization and/or crisis intervention. Meet with and provide information to community agencies, landlords, area businesses, law enforcement agencies, and the general public to increase community awareness of mental illness and availability of services. Participate in resolving crisis situations involving assigned caseload/clients to ensure continuity of care and consistent resolution with care plans. Minimum Qualifications Bachelor’s Degree in one of the Social or Behavioral Sciences and four (4) years of experience in professional community mental health work. Master’s Degree in Social Work may be substituted for two (2) years of experience. Equivalent work experience may be substituted for education on a year‑for‑year basis. Benefits 13 paid holidays, 12 days of sick leave, and 3+ weeks of vacation leave annually (vacation leave accrual increases with years of service). Health Insurance Coverage: State pays 85–100% of employee‑only premiums (annual value $12,845.82–$15,112.76). Health Insurance Premium Credit: Participation decreases employee‑only premiums by 5%. Dental Insurance: State pays 100% of employee‑only dental premiums (annual value $395.72). Retirement Plan: State contributes the equivalent of 14.11% of the employee’s pay to the Maine Public Employees Retirement System (MainePERS). Parental leave: Six weeks of fully paid parental leave for employees welcoming a child (including fathers and adoptive parents). Additional unpaid leave may be available under the Family and Medical Leave Act. Additional wellness benefits available through the Office of Employee Health and Wellness.
