Key Responsibilities

As a Physical Therapist at Confluent Rehabilitation, your core responsibilities will include:

• Directing physical therapy treatment plans aimed at preventing disabilities, alleviating pain, and enhancing muscle function.
• Overseeing physical therapy assistants to ensure the implementation of the individualized plan of care.
• Conducting thorough evaluations of clients’ functional abilities using diagnostic tests to ascertain their rehabilitation needs.
• Assessing home environments to identify opportunities for improved independence and recommending modifications as necessary.
• Providing physical therapy services in accordance with physician directives, including the administration of exercises and use of therapeutic modalities such as traction and massage.
• Preparing and maintaining accurate clinical records, including evaluations, care plans, and progress notes, compliant with OASIS requirements.
• Engaging actively in interdisciplinary team meetings to discuss client progress and adjust treatment plans as needed.
• Delivering education and instruction to clients and families regarding treatment strategies and use of therapeutic devices.
• Completing timely documentation for all clinical services rendered and ensuring compliance with agency policies and regulations.

Qualifications

The ideal candidate for the Physical Therapist role will possess the following credentials:

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• Current and valid physical therapy licensure in the state of Virginia.
• At least two years of clinical experience as a licensed Physical Therapist in an acute care or home care setting.
• CPR certification, a valid driver’s license, and proof of automobile insurance are required.
• A strong commitment to clinical excellence and quality care.

Preferred Experience

While a minimum of two years’ experience is required, candidates with additional experience in multidisciplinary healthcare settings, particularly those focusing on outpatient rehabilitation, will be given preference. Compensation and Benefits
