Extended Home Care
Extended Home Care is seeking a dedicated Registered Nurse (RN) to join our Home Care team in New York. You will perform initial patient assessments, develop and implement care plans, and coordinate case management for assigned caseload. The role requires a valid state license, CPR certification, and at least two years in public health or home care nursing.
