Hospice RN Case Manager — Equity Eligible & Growth

UnitedHealth Group is seeking a Registered Nurse Case Manager for Casa de la Luz East Valley, part of LHC Group. You will develop and implement plans of care, collaborate with physicians and families, and ensure high-quality hospice services for patients facing life-limiting illness. The role requires an RN license in the state, valid driver’s license, and CPR (or ability to obtain within 90 days).
