Work Environment\n

This remote position allows for flexibility while maintaining accountability in service delivery. You will work within a supportive and collaborative team composed of healthcare professionals, including nurses, physicians, and counselors, all focused on providing comprehensive care to our clients. While most of your interactions will be conducted through telehealth platforms, you will also have opportunities to engage directly with clients and their families during in-person visits as necessary.

Key Responsibilities\n

• Conduct in-depth psychosocial assessments to understand clients' mental health concerns, strengths, and environmental influences that may affect their well-being.

• Deliver individual, family, and group therapy sessions utilizing evidence-based practices tailored to meet the diverse needs of our clients.

• Employ therapeutic modalities, such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed care, to address psychological and emotional challenges.

• Develop and implement personalized treatment plans that set specific, measurable goals and outline therapeutic interventions.

• Engage in collaborative treatment planning sessions with psychiatrists, psychologists, and other interdisciplinary team members to coordinate optimal care for clients.

• Facilitate crisis interventions, perform risk assessments, and formulate safety plans to ensure clients' immediate safety and long-term stability.

• Document all clinical interactions accurately and promptly, adhering to ethical and legal standards to maintain the integrity of our client records.

• Lead group therapy sessions and psychoeducation classes aimed at bolstering resilience and coping strategies among clients.

Patient Population / Client Focus\n

Our clinical focus encompasses a wide array of mental health conditions, including depression, anxiety disorders, and trauma-related conditions, primarily within hospice settings. You will work with clients of varying ages, experiencing psychological distress related to end-of-life issues, chronic illness, caregiving stress, or grief. The goal is to provide therapeutic support that empowers them and their families during such challenging times.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Colorado.
