Description

• Conduct evaluations for patients in both inpatient and outpatient environments and craft personalized treatment plans
• Implement physical therapy interventions concentrating on mobility, strength, balance, and overall function
• Collaborate closely with nurses, physicians, case managers, and fellow rehabilitation professionals to enhance patient outcomes
• Educate patients and their families about therapeutic objectives, discharge processes, and home care exercises
• Document evaluations, treatments, and patient progress to meet facility and regulatory standards
• Participate in care team conferences and interdisciplinary meetings to ensure coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• Must possess state licensure as a Physical Therapist
• CPR certification is preferred
