Case Manager Job Details:
Schedule: Monday-Friday, 8:30am-4:30pm
Pay Rate: $24/hour

About the Case Manager Role:

  • Make a meaningful difference by helping veterans and their families achieve housing stability and self-sufficiency.
  • Manage a dedicated caseload and develop individualized service plans that create measurable outcomes.
  • Utilize a Housing First approach focused on removing barriers and promoting long-term success for clients.
  • Work collaboratively with community partners, social service organizations, and veteran-focused resources.
Case Manager Job Description:

The ideal candidate is compassionate, resourceful, and highly organized, with the ability to build trusting relationships and empower clients to achieve their goals. Working with a caseload of approximately 15-30 veteran households, the Case Manager serves as a dedicated advocate who helps coordinate services, navigate benefits, and remove barriers to housing stability.

Key Responsibilities:
  • Conduct assessments to determine eligibility, identify needs, and connect clients to appropriate services and resources.
  • Collaborate with clients to develop individualized housing stability plans with measurable goals and outcomes.
  • Provide ongoing support, advocacy, crisis intervention, and resource navigation services.
  • Coordinate with internal teams, community providers, housing resources, and benefit programs to support successful client outcomes.
  • Conduct outreach activities to identify and engage veterans in need of supportive services.
  • Participate in community meetings, agency events, and interdisciplinary team discussions to support program goals.
Case Manager Job Requirements:

Education & Experience
  • Bachelor's degree or equivalent work experience in Social Services, Human Services, Nonprofit Administration, Case Management, or a related field.
  • Minimum of two (2) years of experience in nonprofit, social services, housing services, case management, or related program delivery.
  • Experience working with grant-funded programs, veteran services, housing programs, or low-income populations is highly preferred.

Knowledge & Skills
  • Ability to assess client needs, develop service plans, and support participants in achieving goals.
  • Experience maintaining accurate documentation and ensuring compliance with program and grant requirements.
  • Proficiency with Microsoft Office applications, including Word, Excel, and Outlook.
Additional Requirements
  • Valid, unrestricted driver's license with an acceptable driving record.
  • Current vehicle registration and automobile insurance.
  • Ability to travel within the community to meet with clients, service providers, landlords, and community partners.
